@KH LEE , I actually discussed this with Dr Ahmad, he says he would choose both IANB and incisive, clearly mentioning that its justifiable to choose both, which conveys both could achieve desired anesthesia
Lingual infiltration, although as you mentioned, we use it in some cases in the clinic, is not academic. We prefer to NOT choose it
They could throw in Buccal infiltration as an alternative to Buccal nevre block, in cases where manipulation of the buccal soft tissues (adjacent to the mandibular molars only) is planned as part of the procedure. Example extraction of mandibular molars

1. Inferior Alveolar nerve block.
Bilateral IANB is okay, it is just inconvenient and quiet uncomfortable for the patient. But it is justifiable when needed.
2. IANB for both.
